Organisation Overview: St Luke’s ElderCare (SLEC) is a Christian healthcare provider committed to meeting the evolving needs of the community, excelling in social care and clinical excellence for ageing well. Our ecosystem of care comprises centre, community, residential and home-based services.
We operate 19 Senior Care and Rehabilitation Centres, 12 Active Ageing Centres (Care) and 2 Residences (nursing homes), serving mainly older persons regardless of race, language and religion.
Brief Description: The Community Engagement Associate Intern will primarily be responsible for the coordination and implementation of community engagement programmes and services as well as the management of members and volunteers. Programmes and services are operated in accordance with MOH’s Active Ageing Centre (AAC) service requirements.
Roles and Responsibilities:
Organise and coordinate community engagement programmes
Carry out home visits, care planning, service referrals, etc, to support the needs and well-being of the seniors in the community
Collaborate with internal and external partners to implement programmes and services
Provide administrative support for reporting internally or externally
Learning Objectives and Outcomes:
Discover assets, needs and interests within seniors in communities and learn to design activities around them
Learn how to promote activities and recruit seniors to encourage social participation and combat loneliness
Be involved in the conduct of home visits to engage seniors who might be frail or socially isolated
Witness firsthand and work with professional community and social workers on assisting seniors in cases in real-time through discussions and case management.
Learn how to engage and attend to enquiries related to issues seniors may have
To maintain internal CRM database used to document the organization’s interactions with members and volunteers
To build partnerships and collaborations with schools, corporates, social service and community agencies
Duration of Internship: 6 months
Daily Working Hours: Mon-Fri, 8.30am - 6.00pm
Will this internship require weekend work?: No weekend work required

Will be assigned to one of St Luke’s ElderCare Centre
Working hours may varies with different centres
