Organisation Overview: St Hilda’s Community Services Centre (SHCSC) was registered with the Registrar of Societies and the Charities Act in 2001. We are a VWO, a social service arm of the St Hilda’s Church. SHCSC has worked in the Tanjong Rhu/Old Airport community for more than 24 years, serving 20 blocks of HDB flats. Our services include; Active Ageing, Senior Day Care, Day Rehab , PWID, Home Personal Care and Medical Services Transport with Social, Nursing and Care Teams to care and support the community.
Brief Description: The intern will assist in sourcing financial assistance schemes, subsidies, and funds for clients experiencing financial difficulties. They will provide support services to families and clients, with duties that may include giving care to the elderly with needs across various fields such as Day Care, Community Rehabilitation, Home Personal Care, and Social Work.
The intern will also assist in providing interventions and programmes, performing community development work, and contributing to the enhancement of professional practice in social work.
Roles and Responsibilities:
Assist to Social Worker and her team in :
1. Follow up some casework and home visits where necessary
2. Co-ordinate collaboration between different programs/services with Daycare: Home Care, Medical Escort Transport, Community Nursing, Rehabilitation, Active Ageing Ctr. etc.; to ensure smooth transition of patient care
3. Ensure proper record management for audit purposes
4. Participate and assist in other Community events and activities where relevant
5. Means Testing follow up of docs if necessary for renewal
6. Preparation of documents of clients’ admission
7. Preparation of referral for clients who need multiple health & social care and support services
8. Monitor the trends of clients types, in terms of clients’ subsidy level, service(s) received etc. To identify service gap if any
9. Source for financial assistance scheme/subsidy/fund for clients with financial difficulty
10. Organise multidisciplinary case patient discussion in regular basis
11. Proper documentation and record of case management of clients by SW
12. Stakeholder engagement e.g. media, grassroots etc
13. Involvement in other projects/social activities organized by centre e.g. community outreach
14. Any other duties as may be assigned
Learning Objectives and Outcomes:
1. To gain knowledge and experience in conducting assessment and managing casework and counselling with elderly
2. To learn how to co-ordinate and collaborate with different program services Unit Heads
3. To participate in community events and activities
4. To be exposed to conducting Means Test and/or Financial Assistance
Duration of Internship: 5 months
Daily Working Hours: Mon-Fri, 9.00am - 6.00pm
Will this internship require weekend work?:
Flexible - weekend work may be required depending on operational needs
