[C14] Care Coordination and Community Care Intern (NTUC Health Co-operative Ltd)

Organisation Overview: NTUC Health Co-operative Limited (NTUC Health) is an NTUC enterprise that provides a comprehensive and integrated suite of quality and accessible health and elderly care services to meet the growing needs of families and their dependents. NTUC Health is among the largest senior day care, nursing home, and home personal care providers in Singapore. We also offer other services for seniors such as active ageing centres, community support for vulnerable seniors, and a sheltered senior group home.

Brief Description: You will have the opportunity to gain hands-on exposure to community care coordination within NTUC Health’s Homecare services. You will actively participate in home visits, client engagement, and caregiver support, while supporting service coordination across healthcare and social partners. This role offers direct experience in delivering integrated, person-centred care within the community.

Roles and Responsibilities:

1. Work closely with internal stakeholders to support smooth delivery of homecare services

2. Assist in managing client inquiries and coordinating requested services

3. Participate in case discussions and multidisciplinary care planning

4. Assist in coordinating referrals with internal teams and external community partners

5. Participate in home visits with care teams to observe and support client assessment, care delivery, and follow-up

6. Engage with clients and caregivers during home visits and telephone follow-ups to understand needs and challenges

7. Support caregiver education by reinforcing care techniques, resources, and available support services

8. Collaborate with social and healthcare partners to recommend appropriate care programmes

9. Assist in discharge planning coordination with Operations and Nurse Managers

10. Support the analysis of backend statistical data to identify insights and enhance client and caregiver experience

11. Actively support and participate in shaping development of programme development, evaluation, and service improvement initiatives

12. Contribute to service innovation or pilot projects to enhance care delivery

Learning Objectives and Outcomes:

1. To be exposed to community-based healthcare and integrated care models in Singapore

2. To be able to understand and support client assessment and care coordination processes

3. To apply communication and interpersonal skills when engaging clients and caregivers

4. To participate in home visits and multidisciplinary collaboration across healthcare and social sectors

5. To learn how to support caregivers through education, engagement, and resource navigation

6. To gain knowledge in programme development, service evaluation, and community care delivery

Duration of Internship: 6 months

Daily Working Hours: Mon-Fri, 8.30am - 6.00pm

Will this internship require weekend work?: Occasional weekend work for special events/projects
