Brief Description: The role involves proactively engaging clients, organizing social activities, and promoting active lifestyles to prevent social isolation, while empowering residents to take charge of their journey toward healthier and more active living. The role also entails referring clients to relevant community care and social assistance when needed.
Roles and Responsibilities:
Assist in Active Ageing Committee in development and implementation
Assist in organising and facilitating wellness programmes for elderly clients
Work with colleagues to identify programme needs and make improvements
Liaise with volunteers and trainers to implement programme
Perform community screening for seniors (outreach or referred)
Assist with programme intake and conduct the necessary assessments for clients
Assist and provide training to elderly clients on the use of mobile devices for accessing hybrid activities
Conduct health assessments and screenings for clients as needed
Support colleagues in group work initiatives and outreach activities
Assist in identifying, developing, and maintaining community partnerships for service linkages
Assist the befriender coordinator in befriending clients and conducting assessments
Provide education to the elderly on the management of chronic diseases
Provide administrative support by documenting and updating of client records in the system
Perform any other duties as assigned by the Programme Executive or Manager
Learning Objectives and Outcomes:
Understand and relate to the blueprint of our national initiatives in supporting older persons in the community
Gain real experiences in operationalising eldercare services, such as AAC, and relate intimately to challenges on the ground
TCommunicate and connect with older persons
Duration of Internship: 5 months
Daily Working Hours: 9.00am - 6.00pm
Work Days: Monday, Tuesday, Wednesday, Thursday, Friday
Minimum Qualification: No minimum education (i.e. A-level Graduates, Current Polytechnic students are welcome)
