Organisation Overview: Yong-en Care Centre is a social service agency serving the community through our Core Services: Family Support Services, Dementia Day Care, Home Care Service, and Active Ageing.
Brief Description: Youth
Roles and Responsibilities:
Case Management & Stakeholder Relationship Assistance
Coordinate with other organisations for the execution of programmes and managing of cases
Identify needs & risks for the cases by home visiting and other social work tools in youth cases.
Assist in render interventions and address needs for the cases in timely manner such as home visit and conduct outdoors activities for youth.
Prepare referral letter and case summary for the purpose of youth case referral and case management.
Manage communications and feedback from stakeholders
Programme Management
Coordinator for Youth Programme and assist in running programmes (indoor and outdoor).
Outreach to Community
Conduct outreach activities to target client group, other organisations and the community
Support development of community outreach plan to other organisations and the community
Walk the ground and do the community mapping of the assets of the community.
Develop social and community work with external parties for purpose of publicity & networking.
Assist to share to community groups the mission, vision, purposes, programs, and policies of the centre
Administrative Duties
Attends to public enquiries.
Update filing system and records.
Learning Objectives and Outcomes:
To understand the operation of a youth centre.
To understand the needs of youth.
To understand community in a strength-based perspective and ABCD of a community asset based concept.
To appreciate the risks and needs of youth and the challenges one faces when working with a youth.
Duration of Internship: 6 months
Daily Working Hours: 9.00am - 6.00pm
Work Days: Monday, Tuesday, Wednesday, Thursday, Friday
Minimum Qualification: Polytechnic diploma or professional qualifications
