Organisation Overview: St Hilda’s Community Services Centre was registered with the Registrar of Societies and the Charities Act in 2001. We are a VWO, a social service arm of the St Hilda’s Church. SHCSC has worked in the Tanjong Rhu/Old Airport community for 22 years, serving 20 blocks of HDB flats. Our services include; Active Ageing Centre, Day Care Centre, Day Rehabilitation Centre, Home Personal Care and Medical Transportation Services with Social, Nursing and Care teams to care and support the community.
Brief Description:Provides support to
Conceptualise, organize, promote and execute events and programs for the elderly and residents in the community
Facilitate and conduct daily activities at the Centers or oversee external providers and volunteers
Collate data and statistics for reporting to agencies
Preparation of activities materials, bulletin & promotion flyers
Conduct regular needs assessment of emerging trends for the elderly.
Organize and participate in festival and community events.
Roles and Responsibilities: To Assist in:
Overseeing the preparation and managing of the Centre Programs and Activities, in conjunction with other staff for group and person centered approaches.
Managing & evaluating Quality Improvement (QI) projects / program in the aspects of psychosocial, overall care etc.
Conceptualize, organize, schedule, promote programs for the elderly.
Logistics setup for the running of activities on daily basis.
Facilitating or conducting daily activities for the elderly.
Participants’ attendance during the activities session, on site to monitor the
Safety of the participants and assist the instructor, if any, during the session.
Tracking of transactions arising from the programs.
Preparation of activities materials and bulletins.
Collating of data and statistics for reporting.
Walk in enquiries, phone call, and grocery ration distribution onsite/delivery to clients’ place.
Wheel angels whenever the need arise, for residents to and fro home and Centre for activities.
Sterilizing & clean therapy equipment for infection control.
Observing, monitoring and recording clients' conditions by taking temperatures, pulse, respirations and weight.
Communication with clients, relatives and carers.
An any other duties as may be assigned.
Learning Objectives and Outcomes:
To be exposed to serving the elderly and vulnerable in the community.
To be able to conceptualize, organize and execute events and programs for the elderly.
To apply communication skills to the elderly
To learn how to organize and participate in festival and community events.
Duration of Internship: 6 months
Daily Working Hours: 9.00am - 6.00pm (Mon - Fri), 9.00am - 1.00pm (Sat rostered duty )
Work Days: Monday, Tuesday, Wednesday, Thursday, Friday, Saturday
Minimum Qualification: No minimum education (i.e. A-level Graduates, Current Polytechnic students are welcome)
