Today, Montfort Care offers a wide range of programmes through family service centres, a child protection specialist centre, eldercare services, Active Ageing Centres, an integrated caregiving hub, the National Anti-Violence and Sexual Harassment Helpline (NAVH), and mental health support services.
Brief Description: Advanced Care Planning (ACP) Support
Roles and Responsibilities:
Shadow and assist in ACP discussions with seniors, families, and healthcare professionals, gaining insight into decision-making processes for end-of-life care.
Support ACP initiatives, including preparing materials, organizing discussions, and assisting in follow-up actions to ensure that senior citizens’ wishes are clearly documented and respected.
Assist in potential ACP outreach events, helping to organize and promote events aimed at raising awareness and providing information about ACP to seniors and the community.
Learn from professionals in healthcare, ethics, and gerontology, observing the critical conversations around end-of-life care and becoming familiar with the ACP process.
Learning Objectives and Outcomes:
To gain in-depth understanding of ACP, with opportunities to observe and contribute to conversations that have a profound impact on seniors' well-being and dignity.
To be exposed to networking with healthcare professionals and the ethical considerations involved in senior care, enriching the knowledge of gerontology and psychology.
To gain knowledge and hands-on experience in facilitating discussions and preparing materials related to sensitive and important healthcare decisions.
To gain experience in community engagement, event organization, and raising awareness around vital senior care topics with the involvement in ACP outreach events.
To gain strong interpersonal and communication skills development, as the intern will engage directly with families and seniors in critical care decisions.
Duration of Internship: 6 months
Daily Working Hours: 9.00am - 6.00pm
Work Days: Monday, Tuesday, Wednesday, Thursday, Friday
Minimum Qualification: Polytechnic diploma or professional qualifications
